(Less than 18 years of age)
All employment certificates may be secured from the Middle School Guidance Office. According to state law, an employment certificate cannot be issued until the following procedure is completed:
- A parent, guardian or legal custodian must apply in person with the minor.
- A written Promise of Employment form must be completed by the employer, parent and school doctor.
- Proof of age must be supplied in the form of a birth certificate or baptismal certificate.
PUPIL RECORDS
Information for pupil records is collected and maintained in accord with federal guidelines for privacy and access. If a student transfers to another school district, his/her permanent record, testing record and health record are forwarded upon receipt of written notification of admission. High school transcripts are released only upon written authorization from a parent or eligible pupil.
REPORT TO PARENTS
Sharing the progress of pupils with parents is done every nine weeks with a report card. Also, progress reports are mailed home to parents at the middle of the nine-week period for those students who are having difficulty. Students who have demonstrated exceptional progress may also receive a report of commendation.
WITHDRAWAL PROCEDURE
Elementary: Pupils moving from Milton Area Schools should inform the school office at least three days in advance.
Secondary: Any pupil who plans to move or withdraw from school should discuss this with a guidance counselor as soon as possible. On the last day of attendance, the pupil must report to the guidance counselor and complete all school obligations before final withdrawal approval.
Temporary Withdrawal Due to Pregnancy: The Milton Area School District encourages girls who are pregnant to continue with their education; however, temporary withdrawal from school may be medically necessary. The following procedure shall be followed for a temporary withdrawal:
- Student notifies her guidance counselor and makes arrangements for temporary withdrawal.
- Student contacts teachers to obtain assignments.
- During the period of absence, it is the student's responsibility to return completed work to teachers and to pick up additional and continuing assignments. All assignments must be satisfactorily completed.
- When the student is able to resume her full-time schedule, she obtains a "Temporary Withdrawal Permission Return Slip" from her counselor and presents it to each teacher for approval.
